NEW PATIENT INFORMATION

SCHEDULING AN APPOINTMENT

We are honored that you have chosen Beyond Therapy. Please read the following information carefully, and feel free to reach to our clinic if you have any questions or concerns. The following items are needed prior to scheduling:

  • Prescription: We must have a script signed by your physician for therapy. If you have the script, please email it to bt@beyondtherapy.care or fax it directly to 979-743-2109. â€‹â€‹

  • Completed forms: This includes medical history, consent forms, and sensory profiles (if requested by therapist).

​

  • ​Medical History and Consent forms can be found online through the patient portal. To set up your patient portal, please contact our office with your email information. â€‹

​

  • If you are a returning patient or need to log in to your account for any other reason and need your password reset, please contact our office. 

​

​Insurance Information:​

  • Updated copy of your insurance card.

​​

  • Most HMO plans require that a referral be submitted to your insurance by your referring doctor. If that is what your plan requires, please contact your doctor and let them know so they can initiate this process for you.​
